Lower Eyelid Blepharoplasty — The Transconjunctival Approach
The transconjunctival approach is often considered for younger patients or those with relatively good lower eyelid skin tone and mild laxity, but with noticeable lower eyelid fat prolapse (“under-eye bags”).
One of its advantages is that there is no external skin incision beneath the eyelashes and no visible skin sutures along the lash line. Instead, the incision is made inside the lower eyelid, through the conjunctiva, providing access to the underlying fat compartments.
In selected patients, a tied-down bolster may be used after surgery to provide gentle, temporary support to the lower eyelid during the early healing period. This can help maintain the desired eyelid position while the tissues heal.
The goal is a smoother, refreshed lower eyelid contour while preserving the natural appearance and function of the eye.
